The Arizona Department of Health's mission is "monitoring the health, safety and welfare of children in child care centers licensed and certified group child care homes throughout Arizona, by establishing appropriate standards, monitoring compliance, providing technical assistance and training caregivers and provide consumer education. "To live in this mission, a strict inspection and monitoring is done when a person, association or corporation applying for a license to operate a child care center. This is the objective of ensuring that the best possible care will be given to children in the state that will be under the care of these centers, while their parents are at work or other economic activities for their livelihood.
License application
The applicant for a license to operate a child care center in Arizona must be at least 18 years of age and a U.S. citizen or legal resident and must be a resident of Arizona. If the applicant is a corporation, partnership or limited liability company must be a national entity or a foreign entity that is qualified to doing business in Arizona. If the applicant is a partnership, one partner must be a U.S. citizen or legal resident alien and must be a resident of Arizona.
Here are some requirements to be submitted in connection with the application for a license to operate a child care center at home:
 • Copy of a valid class of applicant or class two fingerprint clearance card;  • Background Affidavit Criminal Class I or Class II;  • Certificate issued by the Department showing that the applicant has completed at least four hours of training provided by the Department including the Department's role in licensing and regulating child care centers under the Arizona Revised Statutes;  • If the applicant is a person, a copy of proof of U.S. citizenship or legal resident alien;  • If the applicant is a corporation or a limited liability company, a certificate of good conduct issued by the Arizona Corporation Commission dated six months before the date of application;  • If the applicant is a partnership or a partnership, a copy of proof of U.S. citizenship or legal resident of one of the partners or members of the association;
The Department issued a license to the applicant, depending on the integrity of the application and other requirements. The Department will notify the applicant of any deficiency in the application and other requirements, giving you the opportunity to complete or correct the deficiencies.
As part of the substantive review of the application of a license, the Department conducted an inspection that may require more than one visit to the facility. The Department issued a license or approval if that the applicant and the facility are in substantial compliance with all requirements of the Department of Health Services and the Arizona Administrative Code.
